Name: (someone told me it had to be written in kanji, hiragana or katakana?)
Age range: choose the appropriate one
Gender: male 男 female 女
List 5 favorite artists
List 5 favorite songs
Name the "break out" artist of 2010
Name special projects you would like to see in the future: (For example: hit medleys or artist collaborations)
*NOTE: ALL fields must be filled in or the form will not be accepted.*
Click on LEFT button (at the bottom of the page to submit your votes)
You will see a page with all your choices. If it looks good, then click on the TOP button.
You'll see this when your vote went through ^_^
and you are done ^^
